    It seems like yesterday when the rapidly accumulating experience with the StratusTM OCT in a variety of
    macular disorders and glaucoma led us to share our experiences with the readers in the 2nd Edition of “Optical
    Coherence Tomography of Macular Diseases and Glaucoma”. While the StratusTM OCT provided a unique
    capability of seeing through the various layers of retina (in vivo Histopathology) as never before and empowered
    the retina specialist to answer some of the most challenging questions and make management decisions that
    were not possible before, it soon became apparent that the ‘Time domain’ technology, that the instrument
    used, was slow, was sensitive to unclear media, did not scan in real time and hence did not give truly reproducible
    results. Further, it was not capable of generating 3-D image reconstructs, and did not have the capability to
    segment each layer of retina separately.
    While some years ago we were happy to see, what were rather gross morphological changes, the rapidly
    advancing OCT technology will soon provide confluence of the microstructure and the functional biopsy of
    macula. Currently, a number of OCT machines are available, practically all based on ‘Spectral Domain’
    technology to allow faster scanning rates typically 25,000 to 40,000 scans/second. High-resolution images are
    obtained in real time, which have ‘point-to-point’ registration with the fundus image obtained with a confocal
    scanning technology. Thus, it has become a remarkable tool to follow structural changes not only in patients
    with macular disease but importantly in glaucoma as well.
    While these are exciting times in the field of OCT technology and its applications what with new information
    becoming available practically everyday, we would like to share our early experiences obtained in the last 2
    years of using the new ‘Spectral Domain’ OCT in this thoroughly revised 3rd edition of the book.
    We do appreciate the concerns and apprehensions of our colleagues who have invested heavily in the ‘Time
    Domain’ StratusTM OCT, that with the availability of next generation technology, the current machines would
    soon become obsolete. We believe that StratusTM OCT would remain relevant for the next few years what with
    all the multicentric trials using this machine for tracking changes in macula. Moreover, time domain machine
    would continue to provide useful morphological information to the clinicians and will continue to play important
    role in the day-to-day management of patients with macular pathology. Keeping in view their requirements,
    we have retained many of the StratusTM OCT scans from the previous editions wherein we feel that SD OCT
    does not offer any greater advantage over the StratusTM OCT scans.
    We would like to express our heartfelt gratitude to Carl Zeiss for giving us the opportunity to use CirrusTM
    HD OCT under their early access program. We highly appreciate the contributions of our technicians Ms
    Reena Kumari and Ms Nirmala Kumari for obtaining some of the most outstanding OCT scans used in this
    book. We also acknowledge the contributions of our ophthalmic photographer Mr Arun Kapil, easily one of
    the best ophthalmic photographers in the world today, who has contributed all the fundus photographs and
    fundus fluorescein angiograms used in this edition of the book.
    We are grateful to Dr Pawan Gupta, formerly senior resident at the Advanced Eye Center, PGI, Chandigarh
    for collecting the images from spectral domain OCT and helping us in writing several chapters in the book.
